CERF & Gonioscopy Clinic-

The CERF eye exam is a general eye exam that screens for genetic and non-genetic abnormalities in dogs.  For more information:  http://www.vmdb.org/cerf.html

CERF FORMS - HELP SAVE TIME!
If you would like to have your CERF form mailed to you in advance, include an 8 1/2 x 11 self addressed envelope with sufficient postage along with your registration form. CERF forms will be mailed to you on 8/1/2010 along with your appointment information. Bring the completed CERF form with you to the clinic.

Gonioscopy evaluates the eye to determine if there is increased genetic risk of developing glaucoma.

Clinic fees do NOT include CERF registration.  Owners will be provided a copy of their CERF results and can register their dog by mailing the form to CERF with the registry fee. 

Plan to arrive 30 minutes PRIOR to your CERF appointment time since dog's pupils need to be dilated prior to seeing the vet.

 

CARDIAC Clinic -

Heart  auscultation  will  be  performed for OFA clearances.  Information regarding OFA Cardiac testing can be found at: http://www.offa.org/cardiacinfo.html

Clinic fees do NOT include OFA registration.  Owners will be provided a copy of their Cardiac auscultation results and can register their dog by mailing the form to OFA with the registry fee. 

Echocardiograms * FULL * -  will also be offered.  The price for  echocardiograms includes auscultation.  Owners will receive a full printed color report of the examination that can be used to demonstrate to other breeders or future pet owners the results of the examination.  Sample Report

Echocardiograms are NOT for dogs with pre-existing heart conditions.

SCHEDULING

¨ Your appointment time will be mailed to you after the registration closing date (8/01/2010)

¨ Plan to arrive 15 minutes prior to your appointment to check in and verify or complete any paperwork.  If registered for CERF, you need to arrive 30 minutes prior to your appointment time.

¨ Appointment times are scheduled on a first-come, first-serve basis.  If you are planning to schedule multiple services please register early or your appointment times may not be together.

¨ Your appointment may be scheduled earlier or later than your requested time depending on registration enrollment.

¨ Clinics can be cancelled if the minimum number of dogs is not reached by 8/01/2010

 

 

 

 

REGISTRATION INFO - Read Carefully !

¨ Deadline for registration is August 01, 2010.

¨ Complete a separate registration for EACH dog.

¨ If you are not an ODTC member, please add the additional $5 fee per dog to your total registration cost.

¨ Include a self-addressed stamped envelope with your registration (include an 8 1/2 x 11 return envelope if you want your CERF form mailed to you in advance.) Appointment times and forms will be MAILED to you after 8/01/2010.

¨ Make checks payable to Oriole Dog Training Club or ODTC.

¨ Mail registration to:  ODTC Health Clinic, Janet Kaplan, 5401 Linton Road Road, Sykesville, MD 21784

REFUND POLICY

There will be NO refunds after the 8/01/2010 closing date for CERF, Gonioscopy or Cardiac Auscultation exams. 

Blood Test - Cancellations for Blood Test after the 8/1/2010 closing date will be subject to a $20 processing fee per dog and any remaining balance will be refunded.

Echocardiograms - Cancellations for Echocardiograms after the 8/01/2010 closing date will be subject to a $35 processing fee per dog, and the $140 balance will be refunded. 

Refunds will be mailed within 2 weeks after the 8/15/2010 clinic.

WHAT TO BRING TO THE CLINIC

BLOOD TEST - Bring a self-addressed stamped envelope so that your blood work results can be mailed to you.  If you do not provide an envelope, a $3.00 shipping and handling fee will apply.

CERF FORMS - HELP SAVE TIME!
If you would like to have your CERF form mailed to you in advance, include an 8 1/2 x 11 self addressed envelope with sufficient postage with your registration form. OFA Thyroid & Cardiac Clinics  - If you registered for these clinics, the OFA form will be mailed to you on 8/01 along with your appointment information.  Bring the completed OFA form with you to the clinic. 

¨ If you want to register your dog's Thyroid results, bring a check or money order payable to OFA for $7.50 OR complete the credit card section on the OFA form.  The registration fee needs to be sent with the blood to the lab.  The OFA will send a certificate to the owner in 4-6 weeks.
