dog.
¨ If you are not an ODTC member, please add the
additional $5 fee per dog to your total registration cost.

Include a self-addressed stamped envelope with your
registration (include an 8 1/2 x 11 return envelope if you want your CERF
form mailed to you in advance.) Appointment times and forms will be
MAILED to
you after 8/01/2010.

Make checks payable to Oriole Dog Training Club or
ODTC.

REFUND POLICY
There will be NO refunds
after the 8/01/2010
closing date for CERF, Gonioscopy or Cardiac Auscultation exams.
Blood Test
- Cancellations for Blood
Test after the 8/1/2010 closing date will be subject to a $20 processing fee per dog
and any remaining balance will be refunded.
Echocardiograms - Cancellations for
Echocardiograms after the 8/01/2010 closing date will be subject to a $35 processing fee per dog, and
the $140 balance will be refunded.
Refunds will be mailed within
2 weeks after the 8/15/2010 clinic.
WHAT TO BRING TO THE CLINIC
BLOOD TEST -
Bring a self-addressed stamped envelope so that your blood work results can be mailed to
you. If you do not provide an envelope, a $3.00 shipping and handling fee
will apply.
CERF FORMS -
HELP SAVE
TIME! If you would like to have
your CERF form mailed to you in advance, include an 8 1/2 x 11 self addressed
envelope with sufficient postage with your registration form. CERF forms
will be mailed to you on 8/1/2010 along with your
appointment information. Bring the completed CERF form with you to the
clinic.
OFA Thyroid
& Cardiac Clinics -
If you registered for these clinics, the OFA form will be mailed to you on 8/01 along with your appointment
information. Bring the completed OFA form with you to the clinic.

If you want
to register your dog's Thyroid results, bring a check or money order payable to OFA for $7.50
OR complete the credit card section on the OFA form. The
registration fee needs to be sent with the blood to the lab. The OFA will send a
certificate to the owner in 4-6 weeks.
