Brainstem Auditory Evoked Response (BAER) is the only accepted method of testing for congenital deafness. One test suffices for the lifetime of the animal.  For  more  info on BAER: http://www.offa.org/deafinfo.html

Clinic fees do NOT include OFA registration.  Owners will receive a copy of their BAER results and can register their dog by mailing the form to OFA with the registry fee.

Optigen

Optigen testing provides DNA based diagnosis and information about inherited diseases in dogs.  Go to the Optigen website http://www.optigen.com to view the complete list of genetic test and cost.  A 20% discount will be provided to their listed fees when you use the clinic discount code.  The clinic discount code will be provided at the close of registration (8/15/2009).  You can receive an additional 5% discount if you register online.     The clinic fee charged for Optigen covers the cost to draw and ship the blood to Optigen.  Results will be mailed to the owner directly from Optigen.

CBC/Chemistry/Thyroid/Titers - General Information

 CERF & Gonioscopy Clinic-

Dr. Jennifer Hyman MA VMD DACVO

The CERF eye exam is a general eye exam that screens for genetic and non-genetic abnormalities in dogs.  For more information:  http://www.vmdb.org/cerf.html

Gonioscopy evaluates the drainage angle of the eye to determine if their is increased genetic risk of developing glaucoma.

Clinic fees do NOT include CERF registration.  Owners will be provided a copy of their CERF results and can register their dog by mailing the form to CERF with the registry fee. 

Plan to arrive 30 minutes PRIOR to your CERF appointment time since dog's pupils need to be dilated prior to seeing the vet.

SCHEDULING

¨ Your appointment time will be mailed to you AFTER the closing date (8/15/2009)

¨ Plan to arrive 15 minutes PRIOR to your appointment to check in and verify/complete any paperwork.  If registered for CERF, you need to arrive 30 minutes prior to your appointment time.

¨ Appointment times are scheduled on a first-come, first-serve basis.  If you are planning to schedule multiple services please register early or your appointment times may not be together.

¨ Your appointment may be scheduled earlier or later than your requested time depending on registration enrollment.

REGISTRATION INFO - Read Carefully !

¨ Deadline for registration is August 15, 2009

¨ Complete a separate registration for for EACH dog.

¨ If you are NOT an ODTC member, please add the additional $5 fee per dog to your total registration cost.

¨ Include a self-addressed stamped envelope with your registration. (Appointment times will be MAILED to you 8/15/2009)

¨ Make checks payable to Oriole Dog Training Club or ODTC.

¨ Mail registration to:  ODTC Health Clinic, Janet Kaplan, 5401 Linton Road Road, Sykesville, MD 21784

¨ There will be NO refunds after the 8/15 closing date except for Blood Test.  Cancellations for Blood Test after the closing date will be subject to a $20 processing fee per dog; the balance will be refunded.

WHAT TO BRING TO THE CLINIC

BLOOD TEST - Bring a large self-addressed stamped envelope so that your blood work results can be mailed to you.  If you do not provide an envelope, a $3.00 shipping and handling fee will apply.

OFA Thyroid & Cardiac Clinics  - If you registered for any of these clinics, the OFA form will be mailed to you on 8/15 along with your appointment time.  Bring the completed OFA form with you to the clinic. 

¨ If you want to register your dog's Thyroid results, bring a check/money order payable to OFA for $7.50 OR complete the credit card section on the OFA form.  The OFA will send a certificate to the owner in 4-6 weeks.

¨ Owners will be provided a copy of their  Cardiac results and can register their dog by mailing the form to OFA with the registry fee.

 

Cardiac Clinic - Dr. Risa Roland DVM, DACVIM

Heart  auscultation  will  be  performed for OFA clearances.  Information regarding OFA Cardiac testing can be found at: http://www.offa.org/cardiacinfo.html

Clinic fees do NOT include OFA registration.  Owners will be provided a copy of their Cardiac results and can register their dog by mailing the form to OFA with the registry fee. 

MICROCHIPS

Microchipping will be provided by Golden Retriever Rescue Education & Training.  AVID (American Veterinary Identification Device) microchips will be used.  For more information:  http://www.avidid.com

OPTIGEN - If you paid for your Optigen test online, bring the completed Optigen form and payment receipt with you to the clinic.  If you did NOT pay for your test online, bring a completed Optigen form AND a check/money order payable to Optigen for the amount of your test.

MASSAGE

Canine massage will be available from Anne Hennessey SAMP, ESMT, BS.  Massage services are available for dogs of all ages with special interest in working and performance dogs.  For more information:  http://www.pasmassage.com
